Aldosferone Responses to Hyperkalemia in Healthy Elderly

چکیده
Plasma aldosterone levels are reported to be lower in healthy elderly individuals compared with young individuals, a difference exaggerated by sodium depletion or upright posture. The aim of this study was to determine the aldosterone response to increases in serum potassium with advancing age. In the Clinical Research Center, six healthy young (20 to 35 yr of age) and six healthy elderly (65 to 85 yr of age) subjects underwent evaluation of their aldosterone responses to potassium infusion (0.5 mEq/kg over 45 mm). Both young and elderly subjects had similar basal serum potassium levels (4.3 ± 0.2 versus 4.4 ± 0.1 mEqIL), similar sodium and potassium excretion amounts and similar increase in serum potassium levels during infusion (to 5.0 ± 0.2 versus 5.1 ± 0.1 mEq/L). However, elderly subjects had lower basal levels of plasma aldosterone and a blunted aldosterone response to potassium infusion (P < 0.05, analysis of variance). Advancing age is characterized by relative hypoaldosteronism in the basal state as well as in response to hyperkalemia. This may contribute to an increased susceptibility to hyperkalemia if other potassium regulatory systems fail.

Purpose: This study investigated the effect of endurance activity on brain-derived neurotrophic factor(BDNF), insulin-like growth factor 1(IGF-1) and its binding protein 3(IGFBP-3) in elderly healthy individuals. Materials and Methods: Eleven healthy old males (mean age of 68 ± 2.31 years old, height of 177 ± 3.1 cm and weight of 79 ± 1.5 kg) were studied. Seventy two hours after maxi...

Gordon syndrome is a familial autosomal dominant syndrome with hyperkalemia, increased extracellular volume, hypertension, normal kidney function, and metabolic acidosis. It is also called type II Pseudohypoaldosteronism due to usually normal aldosterone and low renin levels. In this report a 35 years old woman is presented with 2-year history of hypertension, eclampsia, and fetal death. Labora...
متن کاملMild hyperkalemia and low eGFR a tedious recipe for cardiac disaster in the elderly: an unusual reversible cause of syncope and heart block
Hyperkalemia affects the myocardial tissue producing electrocardiographic abnormalities, such as prolongation of the P-R interval, tall peaked T waves, a reduction in the amplitude and an increase in the duration of P wave, and atrial and ventricular arrhythmias, including variable degree heart blocks. Elderly patients are particularly predisposed to developing hyperkalemia and the associated a...
متن کاملArrhythmias and ECG changes in life threatening hyperkalemia in older patients treated by potassium sparing drugs.
BACKGROUND Severe hyperkalemia is a life threatening condition that can cause fatal rhythm disturbance and terminal heart arrest. The most common cause of hyperkalemia in older patients is that of iatrogenic medication-related etiology due to associated polymorbidity, polypharmacy and reduced reserve metabolic capacity. The aim of this paper is to increase awareness in the clinicians of the ris...

IMPORTANCE Spironolactone has been shown to be an effective treatment option for hormonally mediated acne but can cause hyperkalemia. The prevalence of hyperkalemia among healthy young women taking spironolactone for acne is unclear. OBJECTIVE To measure the rate of hyperkalemia in healthy young women taking spironolactone for acne or for an endocrine disorder with associated acne. DESIGN, ...
